Either your regular doctor or the ER. The important thing is that you do it *immediately*! If you have been bitten by a rabid animal the window of time where shots will prevent infection is fairly small.
You need to keep in mind that infection is terminal!!!

Do NOT get vaccinated.
A vaccinated person is MORE likely to get a disease than a non-vaccinated person. The whole theory of vaccination is flawed. It causes a weakening of the immune system thus making those who are innoculated more susceptible to disease.
There are so many awful side effects to vaccination that it should be considered extremely dangerous.
Just sit back and think for a while.
Is there any sense in injecting a disease directly into your body.
We have been subjected to an awful mind control program to enable the drug manufacturers to make a fortune.

If you go to the vaccination liberation web page, at

You will find all the forms necessary to provide exemption for your child.

Yes, your regular doctor would be the place to go to start treatment if you suspect that you were bitten by a dog that did not have it's shots. They may put you into a hospital as the shots do have some side effects. (nothing serious)

Yes usually you do, however you need to call them ahead of time so they can order the vaccine since it isn't commonly kept in stock. Some doctors won't order it for one person and will refer you to a clinic, workplace health or a hospital.

The health department would have the vaccine. I think it is several shots. Be sure you need it first.

For your pet yes, but if you want one for yourself, I think you have to go to the health department in your area.
