Its been about 4 years since you took it out, so it should be FULLY healed(meaning you might see a scar but are not able to reinsert jewelry or find much evidence of a still existing hole) Some piercings never close and other become obstructed with bacteria, dead skin and other substances causing it to not heal at all or correctly. Also, you have to keep areas clean when you are trying to close a healed piercing. Chances are its healed as most piercings on genitals close around 6-12 months fully. Mine healed up in 6 months after having it pierced for a year and a half.

Getting it re-pierced will hurt more than before granted the fact that the healed up area may have scar tissue. The tissue is often harder to pierce because its thicker and tough, thus causing more pain, but its not unbearable. If you go to a good piercer, you shouldn't have a problem.

I would say after four years if it's not closed, it's not going to. If it hasn't closed, then no need for a new piercing.
