i use to work at a doctors office this is the format we used
(put doctors practice / name here)
Please excuse ______(your name here write it in with pen)__from Work/School(circle the one you need with pen)he/she(circle the one you need with pen) was seen in our office today _(write date here with pen)___2006. If you have any questions please contact us at (555)555-5555(don't worry they don't call just looks better)
