Everyone is taking this literally. I think the phrase is just a way of saying keep yourself covered. Like you never know what can happen so always "have you ducks in a row".

It simply means that if you were to have an accident, say an auto accident, and your underwear was not clean, (e.g. skid marks), you would be in an embarrassing situation if nurses or doctors had to remove your clothes to treat your injuries. It's used tongue-in-cheek because, of course, that would be the least of your worries at that point.

Well, it's not the most clear version of the phrase that I've heard, but the concept goes along this line:

If you are going to have an accident (i.e. where you have to be taken to hospital), make sure you are wearing a clean pair of undies - in case anyone has to see them.

Stupid phrase really as if you get hit by a car or the likes, chances are you're going to crap yourself somewhere along the way :)

I think it was a way for parents to teach there children good hygiene. If they thought they were going to be embarrassed by having dirty underwear on in front of the paramedics.

that's why I don't wear any at all. if I have an accident,I don't want to have dirty ones on.
