medical billing laws?

Question

When did the law go into affect for leagl collections of medical bills? How far back can a collection company collect. Is there a statute of limitations on billing? What if the provider never billed your insurance, am I responsible for it even though I provided them with the insurance information to bill too? I am being hounded for bills as far back as 2000 by a hospital and taken to court by another. Is it fair to go back that far?? Please Help me
