How to File
The alien must file USCIS Form I-131, Application for Travel Document, complete with supporting documentation, photos and applicable fees. This form can be downloaded from our Forms and Fees page. See the application for specific filing instructions.
Here are the instructions:
How to File For a Re-Entry Permit
If the alien is a permanent resident or conditional resident, he or she must attach:
A copy of the front and back of the alien registration receipt card (Form I-551); or
If he has not yet received his alien registration receipt card, a copy of the biographic page(s) of his passport and the visa page of his passport indicating initial admission as a permanent resident, or other
evidence that the alien is a permanent resident; or
A copy of the approval notice of a separate application for replacement of the alien registration receipt card (Form I-797) or temporary evidence of permanent resident status.

You want Form I-131, Application for Advance Parole.
However, be extremely careful with using this form. If you have accrued ANY unlawful presence (especially more than 180 days), you will be subject to a three or ten year bar if you leave the United States...even if USCIS grants you a travel document!
Just because they grant the travel document does not mean you can travel.

Do a lot of Internet research and also get a good attorney. Do not depend just in attorneys, some knows less than us. Is such a demand for immigration attorney, many claimed to have Immigration expertises and they do not. I have a personal experience!
