Yes...After I got married, I filed with my married name, using my standard SSN. They sent me a letter, saying that they couldn't process my documents, because the name that I was using didn't match the records that Social Security had on file.
It's a simple matter of correcting your name with at your local Social Security office.

I guess only if you've changed the number? If you've changed your name recently, you should apply for a new card. If you've had to get a new number because of id theft, I would get a new card. You might have to submit a copy of that card for tax purposes. I would check the IRS website or Social Security website for any other info, or just ask a tax consultant - they'll be popping up everywhere in a month or two.

Yes. I suggest that you go immediately WITH THE LETTER to your local Social Security Administration office. You may be the victim of Identity Theft. I also suggest you bring your birth certificate AND a state issued ID with you as well. A copy of the birth certificate will NOT do.
