I would like to know whether or not I am corretly filling out a NYS W4 form. I have been told I should only have a claim of one for myself by the end of the worksheet, but I end up with two.

I am single with no kids.

Here is how it breaks down by question

A. 1
B. 1
C-G. 0

H. Add A-G: 2

What am I doing wrong?

honestly, it doesn't matter all that much. the larger the number of exemptions, the less they will withhold from your check, it is meant to be a guide for your employer. your claim on your 1040 at the end of the year is the important one, because it is the one that determines what you should have paid in for the year. if you put a lower number on this form you will owe more, and a higher number less. typically the best thing to do is to withhold at the higher rate during the year, and then claim the lower rate at the end so you get more back. If you withhold at the lower rate, it may be a good idea to have an additional amount withheld or a flat percent.

W-4 Form Ny State

First: The W-4 is a Federal form. The NY State equivalent is form IT-2104. Because the state form uses numbers instead of letters, I think you are asking about the W-4. You are filling out the worksheet correctly. Remember, the worksheet is a guide. you are not required to follow it. The first link below is an on-line calculator the IRS provides the estimate the best number of exemptions to claim. It considers more factors and will come closer to matching you withholding to your actual tax.

It is not a W-9, it is W-4. W-9 is a request for tax ID number.
