Fear. I would say lack of education, but unfortunately some of the most educated people in history have been intolerant.
I would break down the fear into several sub-categories, though: fear of the unknown, fear of the partially known, fear of the mistakenly known, and fear of the known (generally tied to fear of loss to oneself).
What is interesting is how many people tie fear in as "fear of the unknown". This is being generous. Sadly, a great deal of intolerance in the world is practiced by those who specifically do know. It is exactly this knowledge, and an evaluation of how the "known" affects their own life, which causes the intolerance.

Ignorance breeds intolerance. People(not all) nowadays do not want to take the time to get to know someone or something who is different than them or do anything that is outside their normal realm. If they don't want to learn about anything new or different then they will always be scared and intolerant of what they do not understand or know.
For instance, a couple of years ago I sat by an older woman at work who was very devout Mormon. I was never scared or intolerant of her, but I asked her a lot of questions about her religion and got some really good answers. She dispelled a lot of misconceptions that I had about the Mormon religion. We had great conversations. And guess what, Mormons are pretty normal people(and this is not meant to be mean or hateful to anyone who practices the Mormon religion). Moral of the story, ask questions and learn!

I think it would be unfair to put the reasons on the ususal one or two things that people like to blame. Sometimes it can be a very strong opinion with pride not allowing it to consider things outside of that view. Other times it can be that the other person is right and to compromise is not showing intolerance but rather abandonment of their own beliefs for the sake of acceptance of others. It doesn't have to be fear or ignorance like so many want to explain it away that easy. There is respect and there is tolerance. They are not always the same thing. I do not tolerate abuse in any form but I respect the fact that many may see things different than me.

The alpha male syndrome - and the fear of losing the place - moving downwards in the equation of things around oneself. Moving upwards is the elating part - that and stagnation also breeds contempt for those below you and hence also a level of intolerance. It could be anything, any or many attributes.

It starts with fear and hate for the unknown. Simple cartooning of human being
That led to a domino effect. People like rush on the radio push that hate no more different then the hate the president of Iran is doing there. It turns people to a simple-minded thug that do more stupid things.

Ignorance... If people don't form their own opinions and listen to what others say instead of actually researching every aspect of whatever, they live in denial... Denial = Fear ... Intolerance is basically the fear of someone else being right and you being wrong. If everyone believe what you believe then your all going to suffer the same consequences or get the same rewards...

Starts early....basically intolerant belief systems...intolerant cultures make for intolerant members of the culture.
By COMPARISON, Americans are easily the most tolerant in the world. This is not to say they are all tolerant...it is only BY COMPARISON.

The complete lack of wanting to learn otherwise. People get so stuck in thier ideas they dont want to learn someone else's. then they teach that to thier children who either continue the same or rebel and form their ownversion. It really is a sad,long, and vicious cycle.

I think it's an evolutionary imperative. Back when we were hunters and gatherers it would have probably been to your best advantage to defend your own group and not tolerate others in your territory. It's a bit outdated today, no doubt.
