I am currently working as an English teacher and my advice for these types of things is this:
Reading: practice reading as often as possible. Read everything you see, signs, advertisements etc. Also, start with easier books and work your way up. Not childrens books but more like young adult. It's not so interesting but you will get a good selection of pretty easy English.
Listening: Watch movies and TV, listen to music, and of course and type of question and answer style listening practice with ESL courses is very helpful.
Writing: Most easily, write a daily diary. It doesn't have to be long or interesting, you just have to write it. Get a friend or a teacher to correct it for you and work on your weak points from there. Also, make email friends. It's not difficult to do and you will have to always write mail to them.
Speaking: This one is either easiest or hardest. If you are outgoing you can always talk with friends. If not, practice alone. You can always listen and repeat something from a DVD or CD and it helps to build fluency. That being said, the best way is to get out and meet people.

Here are some exercises I have used: try to read in English aloud, if you listen to music in English try to make out the words and try to sing along I don´t know why but it is easier to improve pronunciation singing than talking, if you watch movies in English don´t read the sub-titles get used to listening in English ( the same if you have cable watch the shows in English ), also for your pronunciation put a pencil between your teeth ( on top of your tongue ) and read aloud forcing yourself to pronounce every word correctly. Hope it helps, worked for me.

hi!dear friend!you can do several works including studying self-study English teachng books, for example books published by Cambridge University(which includes both grammar&vocabulary study&is categorised in 3 levels including elementary, intermediate&advanced),you have also to listen to radio, such as BBC or RCI(depending to the accent you want to speak with),as much as you can,and at last you should be the best friend of dictionaries(such as Oxford dictionary or Longman),since English has the most words among all the languages!I've been a friend of Oxford Advanced Learner's&I've seen the best results!try it!

Watch children's TV programs that are in english, especially educational programs. A friend of mine who is Guatemalan basically learned to speak english very well by watching "Sesame Street"-- no one around him or at his work spoke english, so that was his only resource at the time.

Just listen to English-speaking radio and TV programs...read English writings (newspapers, magazines)...talk to English language people.
Heck, except for not capitalizing your I's in I'm, and one misspelled word, you type english okay.

Nothing is ever as it seems!
Silent e is EVIL!
Most rules in English don't make actual sense.
contradiction, contradiction contradiction
we are garrulous and verbose as a people and our language proves it! How many words do you really need for car or anything for that matter?
As fast as we talk to you, you sound twice as fast to us...
So slow it down and break it into syllables.
As in learning any language, remember not to relate or translate. Learning english, has absolutely nothing to do with speaking spanish. Don't try and relate our language to yours, just learn English as it is... It sounds strange but it really makes it easier.
