The bible was written over the course of several millennia, the first half consists mostly of the collected short stories and oral history of the Hebrews, the second half was likely written during the period between 30 and 500 AD, by the followers of a religious teacher/prophet who was thought to have been born around the year 0 AD.
Several versions of the bible are currently in circulation, with each sect of christianity publishing its own new version every couple of decades.
In a very real sense the bible is still being written, as every year new editions are being released, and new translations are being found for some of the older texts.

Different parts at different times, the first five books of the old testament are pretty much the Jewish Torah, after that its a collection of jewish prophets writings. Psalms written by King David. Then with regards the N,T. the earliest of the four Gospels was written about 50-75 BC, the latest being written well over a hundred years after Christ. Then their are the epistles written by early church fathers to the christians in different regions. Many of the early fathers lived a life of roaming and preaching so the where is a bit harder to pin down.
All in all a difficult question and one with a full answer much much longer than this but its a bit of an overview.

The books that make up the Bible were written by some 40 authors over the course of several thousand years. Most of it was written in Hebrew (Old Testament), and Greek (New Testament), though some parts of Daniel was written in Sumerian I think. The bible is actually several letters and stories written by many people over the course of many hundreds of years.
A few hundred years after Christ lived on earth, the Catholic bishops of the time, who had the writings, decided to make a book. A great deal of discernment was made over which writings went into the book and which ones were left out.
Several hundreds of years later, the Protestants, who disagreed with some of what the Catholics holy book had to say, omitted some of the books, which leaves the popular version smaller than the original.

The different books of it were obviously written over centuries as history occured over time. I think the first complete bible was written in Greece. Not sure of date but in between the fall of the roman empire and the renaissance I would say.

If you believe the conspiracy theroies, the bible was written supposedly written around 200 ad by the ceasar in power at the time. There was a war when he got into power between, two religions (is it ever anything else???) the one which worships the sun earth moon and stars, and a form of christianity. The Ceasar realised that if he could unite the two religions and rule over them he would become one of the most powerful men of his time. So that what he did!! In order to do this he needed to take what he considered to be the most important parts of the religions and combine them in 1 book aka the Bible. The sun from one religion became the halo of the angels, the powerful matriarch revered in one religion became the virgin mary; reverered but no longer powerful (Well it was the men who changed it!) etc. This became the Catholic religion we know today. As for where this was written it would have been in one of the main ampetheatres in rome with the 'parliament' of the time deciding what to keep and what to eliminate! !

Yea, it was sometime in the 1600s, God sent down the complete King James Version to guide his flock. Those people spoke normal english like us but when they got their copy of the freely distributed Bible, they wanted to talk like him. That is why Shakespeare plays sound so Biblical.
It's all just common sense if you think about it with the guidance of the Holy Spirit. This might not be "scientifically" true(whatever that is suppoeded to mean) but it IS Holy Spirit, religiously, bible-believingly true. Look no further for truth.
