Dry and/or greasy skin due to poor oil composition of the skin. Thick greasy oils clog pores, lack of oil leads to dryness and irritation. You need thin oils to moisturize while dissolving and clearing gunk in your poors. Try fish oil or seafood. 2 tsp fish oil a day or 4 servings of seafood a week. Stick it out for at least 2 months; it will take a long time to replace all your oil. Any effect after 1-2 days is temporary or random; so even if it makes you break out a little at first, you haven't given it a full try yet.
In the short term you can wash and moisturize well, but that will only go so far. Plus excessive washing can be drying and excessive moisturizing can be clogging. Use a small amount of a light moisturizer, made with oil not jelly or grease. Often that means soybean oil or mineral oil. Mineral oil means mined from the ground. So soybean oil is usually better, though mineral oil won't cause too much harm. Clean with soap and water, not a harsh acne cleanser. Even then they only work so well. So you really need the seafood.
Antibiotics aren't really good for bacteria long term, they'll come back in force after. Short term they may help. After you get off them find some kefir with acidophilus listed first or 2nd to replace the friendly bacteria they destroyed. Studies show less illness when you have these bacteria, even outside the stomach in places such as the lungs and elsewhere. You want friendly bacteria to fill the void when the antibiotics stop, not harmful ones.
In the short term you might also try 100,000+ iu retinol vitamin A (a megadose, and too much for normal use) or one of the acne drugs that is similar to retinol vitamin A. It's some minor harm to your organs, but it helps against bacteria on your skin. At least it doesn't have the other long term drawbacks to your skin that antibiotics and many scrubs do.

Firstly, poor you! Acne is a horrible thing to have to live with, and a real confidence killer.
I know you said that over-the-counter products don't seem to work, but I use the Garnier Pure range which works very effectively for me. If this desn't work either, have you thought about going to a dermatologist? They can be pricey, but can offer advice and solutions as to what's best for you. But, remember, any new product you start using will not show results for a month or so usually, so don't feel disheartened too quickly!
In the short term, try using an oil-free foundation on your face, preferably one which is non-pore clogging. One (I think it may be Neutrogena?) even says that it heals spots at the same time as covering them, although I don't know anyone who's tried this out. You can also use concealer on bigger blemishes.

I struggled with acne for years and I finally found the solution that works for me. We have been fooled, tricked, deceived, and just plain lied to. It's time that someone put a stop to it, and everyone learn the truth!
Those of us who suffer from acne have usually been given 2 choices and neither of them work:
1. Over the counter remedies such as cleansers, creams, ointments, etc. These things have been proven over and over again not to work. Why? It does not address the root cause of the problem.
2. Go see a doctor and or dermatologist. They will generally prescribe prescription drugs which include topicals and or antibiotics. This, too, has been proven over and over again not to work. Why? It does not address the root cause of the problem.
I found a step-by-step program that cured my acne and it has never come back. It is amazing and doesn’t use any drugs or doctors. The truth is that acne can be cured…if we know the secret the doctors and drug companies don’t want us to know.

I've been dealing with it for three years now. I know; it sucks. I tried Proactive. Didn't work for me. My mom got me this stuff from Melaleuca (forgot how to spell it). It's called Zap It. I just started. It's working for me. But, I don't know. It all depends on your skin type. Maybe you should try it. If I were you, I'd go to the Melaleuca website and check it out first. It's a three step. There's also this stick called "Quik Stick." You use it 1-3 times a day. It dries out your pimples. But like I said, you should really check out the site and get some more info on Zap It.
I also tried Biore ice Cleanser and the the Biore blemish fighting cleansing cloths. Those worked too. People noticed a difference in my face.

I had systemic acne as a teen and it was horrible. It also runs in my family. The only thing that worked for me was accutane. You have to get it by prescription from a dermatologist. It's a pill that you take over the course of a few months. At first your skin will get worse and extremely dry but if you give it time your end result will be beautiful clear skin. I wish I had pictures I could show you of what my skin looked like before and after, it is just amazing.

OK lets be real, acne's a b**** and it's the worst thing to get over but its not impossible. The best thing out there for it right now is Acutane, but, it is not for everyone that's why it is only obtained by a Dr's prespription. Some people tend to be allergic to the medicine, for a few percentage it does not work for, but usually it does, for everyone, even though after you stop using it the acne comes back, like with every other medicine after the treatment is over. It is really harsh on your body though, for women they can't take contraceptives or have kids due to some serious problems that can occur while taking it, other people experience horrible peeling and dryness.
Apart from all this, i used to have acne, apparently not bad enough for Acutane (according to my dermatologist) but it was still bad (at least i thought it was), so i was recommended this over the counter medicine called "Nature's Cure". They have two medicines, one formulated for men, and another for women, and among these they have the topical creams and face washes as well as pills that work from the inside out. I personally tried the cream and face wash. Since i started using it i have yet gotten another pimple, seriously, my face is progressing miraculously, alot of the blemishes are gone, almost all. Smooth skin, it does dry it a little bit but barely, everyone's complimented on it. A few more weeks and my face will be completely clear. To help with this i also get facials done, everyonce in a while to help with the deep cleaning, which helps alot to remove stubborn clogs and blackheads and so forth.
I know not all of these over the counter medicines work for everyone, like myself that i've tried Proactive and Murad and all those things and i actually got my face worse due to them. But this one worked, see if you can find it in a neighborhood pharmacy or drugstore, or supermarket (which some of them also sell them), or online in their website.
About a diet, the main cause of acne is not your diet, eating chocolate or drinking too much soda, it's an old wife's tale, BUT a good diet will prevent excess oils in the skin which will clog pores and cause breakouts.
The main cause of acne are your body's hormones, we can't fix those with a change in diet. For me i go to the gym about 5 days a week, and excercise, this helps your metabolism break fatty acids that may affect your hormone production, as well as providing an overall well being for the body and get you nice and fit, a little blood to the old pump is sometimes the best remedy for current health problems as well as future undeveloped ones.
Stress is another factor that people a lot of the time don't consider, it does affect your acne because too much stress will trigger hormone production in the body and well you know the rest. Try to take life easily, i know sometimes it's hard but you gotta try, for my stress, excercise wipes it all out, and it usually does for a lot of people so think about it.

This is the advise I gave my step son and it seemed to help him.
I buy Retin-A when I got to Mexico, is suppose to help. You can only use it 2-3 times a week though.
Also, use ivory soap and baking soda to wash your face. The baking soda removes the dead skin cells and it's cheap too. Ivory is just a good cleaner.
Eliminate sugars, sodas, sweets altogether.
Do not touch or pick at your face and keep your hands clean. Change your pillow case all the time too, like everyday if you want.
My friends son benefited from the TV infomercial also.
If all else fails, you may be allergic to something, you may have developed a reaction in the past 2 years also.
Lat resort I would see if your insurance will pay for a dermatologists.

I had a bad case of acne a year ago. It was progessively worse and I was already seeing scarring that I became sooo desperate I finally gave into what my mom's friend suggested. This is going to sound really dumb or strange ('cause I thought so and that's why I didn't try it 'til months later) but ... go to a Filipino store and get a bottle of a beauty astringent called Eskinol (the Lemon one; it's yellow), then go to sales person and ask if they have dalacin-c. Dalancin-c is a pill. So, buy yourself some dalacin-c and eskinol and at home, open up two capsules of dalacin-c and put it into the eskinol astringent (I know it sounds weird but trust me, it works!) and then, shake the bottle, get some on a cotton ball and apply to face. Let dry then apply moisturizer. Every time you use it, shake the bottle.
I know it sounds really stupid but this works. I mean, if you're trying toothpaste on your face, might as well try this.
A bottle of Eskinol costs around $3.00 and two capsules of dalacin-c is around $3.00 as well. That totals $6.00. It's real cheap so give it a try.
