What does the 'removed' mean? Is it a degree of relationship, and if so, how is it determined?

Your first cousin would be your Aunt/Uncle's child. The remove part starts with grands. Your first cousin once removed is your Grandma/Grandpa's brother/sister's children (or your Great Aunt/Great Uncle's kids). Instead of being Grand and Great Grand cousins the become once removed, twice removed and so on.

People argue with me on this one, but it means the following:

My mother has a sister named Sue
Sue has a daughter named Abigail
Abigail and I are first cousins
Abigail has a daughter named Sydney
Sydney is my first cousin (child of my first cousin), once removed (a generation 'lower' than my first cousin)
My children and Sydney will be second cousins.

not positive here....but i think it's when this person becomes your cousin through marriage, and then that couple gets divorced.
