Sometimes you can get gym on prescription, as it were. It won't be completely free but you might get a few free sessions or some time with a personal trainer. Go and ask your doctor about it. Usually it's only for people who have severe health problems though, my dad got it after having a bypass op. You're not going to get it for free if you're just looking to shift a bit of a beer belly.

Depending on your local council. Many local councils offer referal schemes which will give you a heavily discounted membership if you have been referred by a doctor. Your doctor may refer you because of a medical condition or even small issues such as wanting to change your lifestyle or stop smoking can be enough for a doctor to refer you.

usually only if you're very ill/overwight. obesity will get you free memberhsip. also you might get lucky if you have a mental illness - but not everyone does - depends on your local area i think. but the gyms that they give u access to will often be pretty grotty and not swanky private member ones.

No... Even if the doctor prescribes an exercise program, you still have to pay for it...

it depends on your physician and your insurance, most insurances dont cover it because so many people want to do it. however if its worth your time looking into, ask your doctor, or search and see if your insurance covers it.
