urea and creatinine levels will come down depending upon the cause of its increase e.g in severe infection it will come down of its own if the infection is controlled otherwise in renal failure with diet control and ofcourse dialysis.

medicines are to be continued for the causative factor, no medicines further required for reducing blood urea/creatinine levels

Depends on what type of medicine. Remember any drug will always pass through the liver and will add to the workload of the liver . (metabolism (breakdown and/or formation) of substances). Drugs or medicine should not be taken in unless specifically needed, especially with old people because their metabolism are slower due to old age. The liver might not be able to excrete the waste products of metabolism and this may may amass and be harmful.
