It's impossible for any of us to be completely sinless, especially in the world we live in today. That's why it's important to pray 5 times a day, and say "Astaghfir-ul-lah" all the time. "Inna al-salaata tanha 3an el-fahsha'a wal munkar"...prayer will make you control your urges...
The important thing is that you always make sure that your "hassanat" (Good deeds) always outweigh your "sayyi'at" (Bad deeds). 1 hassana is always multiplied by 7, and each sayyi'a is equal to just 1. So increase your hassanat as much as you can so that you can tip the balance to the good side, and keep asking for forgiveness...Allah (swt) is All-forgiving, All-Knowing.

the important point is that what is our feeling and decision about the repeated sin we did. If we are sorry for the past sin and we decided not to do this again, when we ask forgiveness in this stuation Allah is the only one who loves to forgive All the best : )

In my understanding no, but the other important thing is to keep trying, may be at some time the person will quit the sin and not do it again, peace

Allah says "for those that repeat the offence we shall exact due punishment". sorry i cant remember ayat #. but i never forgot this verse.

there was a person in Saudi ho was alcoholic and never stop drinking he was arrested many times and jailed and finally when they arrested him for the last time and they new he would not stop drinking they killed him.
