Specifically, if the wife goes by her maiden name, is she addressed as Mrs. *maiden name*?

I hyphenated my name after marriage although my married name is usually omitted from just about everything I do (unintentionally). Most people, businesses, etc. see the first last name and stop there, but to them I'm still married hence Mrs....

No, Mrs. Maiden Name doesn't work, because she's married to Mr. Such and Such. She's Mrs. Such and Such, but she's Ms. Maiden Name.

As far as what to write in a letter, you can address them by names on the outside envelope
Mr. Such and Such and Ms. Maiden Name
but on the inside, I would try to find some term that covered both of them, like "Dear Friends".

You can still bea called Mrs. or you can go by Ms. Personally I like the Ms. Mrs. sounds too old.

My sister and I ( who are married to brothers) go by X. We get called MRS. Y!!
