First try going through the HR Department to find out what exactly is going on. Someone may have misplaced your filled out and signed paperwork and it may not have reached them. If that doesn't work, you'll need to call the Department of Labor (www.dol.gov) and report them.

Absolutely, first things first, if your company has an HR department that you've been working with, you need to go through them first before you try reporting your boss. There may have been some miscommunication, or else some problem with your job status that would prevent you from being eligible to take 12 weeks of leave.
Also, could your boss be confused as to whether you're taking paid versus unpaid leave? If you've been with your company for at least three months working full-time you should be eligible for unpaid leave.

Does your work have a district manager or an alert line for reporting violations? These I would suggest first.
If you do not have a district manager or alert line I suggest calling the person who handled your FMLA I was in contact with someone during my leave.

Contact your local Labor and Industries office (L & I). If you have been employed for more than 12 months and there are more than 50 employees they HAVE to hold your job for you, unless they were planning on lay-offs before-hand. (This happened to my husband when he was off work for surgery)

Are you sure you qualify for FMLA? There are some rules as to who qualifies, it's not everyone who does.
