Hebrew and Israelite is the same thing.
Israelites are an ancient nation of people who lived around the area which is now Israel, and practiced the religion that is now known as Judaism.
The Israelites were divided to 12 tribes, and lived in a united kingdom, in the land called "Israel". After a major rebellion, the Kingdom was divided to two different kingdoms: "Israel"- the kingdom of the ten rebel tribes, and "Judah"- a small kingdom of two tribes around the city of Jerusalem. Judah was under the rule of the original royal family of the United Kingdom.
Not many years after the separation of the kingdoms, Israel was occupied by the Assyrian empire, and the ten tribes who were in it were exiled and gone. They are now known as "the lost ten tribe", as no one knows for sure what actually happened to them.
Judah was still independent, until it was occupied by the Babylonian empire, and the two surviving tribes (Benjamin and Judah) were exiled to Babylon. Luckily, after a short period of time (70 years, I think) the tribes were released. Some of the "Jews" (Israelites from either the tribe of Judah or Benjamin) came back to Israel, and some of them went to live in other places.
The Jews are the only certain Israelites, but there are several tribes in Africa and Asia (mostly from India) who claim to be the descendants of the other ten tribes.

Black Hebrew Israelites (also Black Hebrews, African Hebrew Israelites, Hebrew Israelites) are groups of people of African ancestry situated mostly in the United States who claim to be descendants of the ancient Israelites. They claim that they and many Africans, and blacks in places like Brazil, Madagascar, and the Caribbean are also descended from the Israelites.
African American and African Caribbean Christianity had long developed a comparison of their experience in the New World with that of the Israelites held in slavery in Egypt, particularly as regards the Book of Exodus. From the mid-eighteenth century a metaphorical embrace of a Hebrew identity was a major element of New World African spirituality. Thus in 1800 Gabriel's slave conspiracy in Virginia identified themselves with the Israelites of the Old Testament, as was Denmark Vesey's attempted rebellion of 1822. However it was not until later in the nineteenth century that an identification as ancient Hebrews developed from identification with them.

A Hebrew is someone who descends from my forefather Abraham. An Israelite is someone who descends from Jacob. Jacobs name was later changed to Israel (Genesis 32:28 kjv 28 And he said, Thy name shall be called no more Jacob, but Israel: for as a prince hast thou power with God and with men, and hast prevailed.) The suffix -ite, means to descend from. So an Israelite is someone who descends from Jacob. Being that Jacob's name was changed to Israel, several times when the bible says "ISRAEL" its NOT speaking about the land, its speaking about the people. Hence, Genesis 35:21 And Israel journeyed, and spread HIS tent beyond the tower of Edar etc. Those who live in Israel today or who claim to be Hebrew Israelites, are actually ashkenazis from Gomer, Noah's grandson. they are not hebrew at all, let alone israelite.

Any one who is a decedent of Abraham is a Hebrew. Any one who is a decedent of Jacob/Israel is an Israelite. In the New Testament the followers of Jesus are spiritually of or decentest from Abraham.
