i've used the cream on areas and it heals in 1-2 days but this patch reappears on other areas. i've changed shampoo to a hypoallergenic one and i've stopped using softener in her clothes..etc.

it could be an allergy to her food...

What cream did the vet give? Sounds a bit like ringworm, and if so, a systemic medication may be necessary.
If it keeps coming back, obviously something's still causing it so you must find out that. As others have said, it could also be an allergy, and that's much more difficult to cure.
Do you have other animals? Cats are especially prone to carrying ringworm, and other skin problems can be easily transmitted among dogs and cats as well.
