Contact your former employer or you can get one from your regional IRS office.

If a taxpayer cannot obtain the documents after attempting to contact the employer, the IRS says the taxpayer should file Form 4852. Form 4852 is the substitute for Form W-2. It is called "Substitute for Form W-2, Wage and Tax Statement, or Form 1099-R, Distributions From Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance contracts, etc.

Contact the former employer's bookkeeping department and make sure they have a correct mailing address for you. W-2 forms for 2006 will not be out until January 07.

Either go to the former employer or contact the IRS and ask for a transcript of your account for the year in question. A transcript of your account shows all w-2 and 1099 forms submitted to the IRS in your Social Security Number.

It should be sent to you at the beginning of the tax season. In case you have moved since you left your former employer, you would have to go to them to request for it.

Ask that employer: If no reponse in 30 days:contact Federal Tax Office. That will put a move on things.

You can only get this form from your former employer. Just call their personnel department.

you can do it one of 2 ways. first is to contact the payroll dept of the company directly. the second is to request it from the IRS it self. i dont know the form number but that is something they can do over the phone, in person at the local office, etc.
