The formal forms of address are Monsignor, Reverend Monsignor or Monsignor [Surname].
Informal forms are Monsignor [First Name], Father, Father [First Name], Father [Last Name], Reverend, etc.
It is always better to ask or be invited to use informal forms of address.

A Monsignor is technically just a priest who has been specially recognized for his work.
"Most Reverend Father" would be an appropriate title for formal introductory purposes.
"Father" would be OK for most other situations.
You may also want to ask the Monsignor how he would like to be addressed.
