My mother toungue isn't English, please help me with writing my name in English

Capitalize the first letter of each name.

Usually, we write "a c".
If it's an official document, sometimes they will ask for the "last name" (family name) first. In this case you write a comma between the family name and the given name ("first name"):

Many of my ESL students make mistakes with this.

Most of the time we don't use our middle names, unless you choose to (because you really like it).
