Usually, we write "a c".
If it's an official document, sometimes they will ask for the "last name" (family name) first. In this case you write a comma between the family name and the given name ("first name"):
c,a
Many of my ESL students make mistakes with this.
Most of the time we don't use our middle names, unless you choose to (because you really like it).
