I am third degree instructor with ATA and just did my last mid-term for 4th degree and I know what you are going through with the board breaks.
You need to remember a mid-term testing is a basically a practice testing for you rank testing. There really is not a pass/fail with a mid-term; you are just going for points.
After my board breaks on my mid-term one of my instructor’s who is a 5th degree gave me the advice for breaking boards of just “slow down and think”. Do not rush the breaks at a testing. If you have done them a million times in class then taking you time and relaxing is the biggest key.

You didn't say what style you are training in, however some tips that should apply to any style are:
1: Breaking is a mind game, first you have to overcome the obstacle in your mind, that's the hard part. Then you have to follow through with the move, making sure to have good form, and the actual breaking part is simple. Just remember mind over matter.
2: Weapons' forms are only different from empty hand forms in that you hold a weapon. The moves are still just memorized patterns. If you are talking about practical weapon usage though, that is a different ballgame. For practical usage, you need to remember to keep your target in mind and the k.i.s.s. method is best (Keep It Simple Student).
3: You have already passed your first degree and are well on your way to your second degree. You would not be in this position if you were not ready and I seriously doubt that your instructor(s) would allow you to delude yourself into thinking you were ready if you weren't. Have faith in your abilities and in your instuctor(s).
Hope that helps. Good Luck!
I was nervous everytime I came up on a promotion and still do.

Hi there A black belt is something you achieve. A Martial Artist is something you are no matter what you're grade is. People train for lots of reasons some train for fitness others for fun but the people that stick at it long term are martial artists. Its just in the blood. I think everyone feels like this when they get their black belt. You would be surprised just how many people quit after putting years in once they reach that level. My only advice is to do some soul searching and look at the reasons why you train? If it's not for the love of the art then there really is no point in continuing. But if you love your training but just aren't getting the buzz from it anymore take some time out and start looking for anther one that my get your juices flowing again. Best wishes idai

Trust me, I know what that feels like! I'm a brown belt in Martial Arts.
Before my belt test, I was totally nervous! I practiced over and over to memorize everything perfectly. But when I did it and got my belt, I knew that nervousness can actually help! It makes you want to practice a lot. If you've practiced over and over on board breaking, weapons and everything else, I'm sure you'll make it! Good Luck!

anxiety is natural. just remember that you have the skills, the ability to use them, and the experience to know when and where during the event.
doubt can be worsened because of adrenaline response. take deep breaths, slow the exhale. relax the body, and remember you can do it. sometimes we over excite our nervous system through playing out the events in our heads, putting the body in the mode, even if we are months away from it.
relax, learn to relax your body too.
you cant have a relaxed mind in a tense body. and vice versa. if one is tense, the other WILL follow.

Well, doubt is normal, but practice practice practice.
I was in martial arts for a while and I learned to be reactionary rather than think too much. Just do it without thinking. When it becomes second nature and instinctual it will be much better.
Good luck!

Be calm and believe in yourself. Do you actually think they would allow you to test if you didn't deserve the belt. Use all of that energy into your board breaking and you will be fine.

Yes, meditate, w/e to clear your mind. Then when it comes the time, close your eyes, take a deep breath, and go for it. Don't think about if your breaking it or not. Don't think, just know.
