I don't think it has much to do with your nipples, you can have rather small breast and have your nipples point down just because they are saggy, I think a reduction is all up to you and your personal preferences, if you have back pain, can't wear the type of clothing you want, or feel uncomfortable, with people (meeting new people talking in front of people that sort of thing) these are signs that a reduction may be what you need, good luck:)

well i'm not no 40DDD(E). But i've heard that women with big breast have a lot of back problems. I don't know if there's any truth to that. But i guess the only way to find out if you need a breast reduction is if you feel comfortable with your breast. If you like the way you look and you feel good don't get a reduction. However, if you are unhappy and you hate them get one.

Most women get reduction for medical reasons like back pain. If you are not having any problems enjoy what nature gave you right. But if they are a problem, talk to your doctor. My nipples point down too I am sure it is not a sign they are too big I am a 40D. A none surgical way of decreasing them is doing exercise. Breasts are mostly fat tissue so if you work out this will help them to go down in size.

The size of your breasts may reduce some if you lost weight. Having a 40 inch back is way too big. Aim for a 34 or 36 at the most. Then see how you feel and look. Talk with a doctor who can evaluate your condition.

I think you need a reduction if you are uncomfortable, in pain or have sever trouble finding clothing to fit. (I myself have pretty large breasts and would like a reduction due to pain, discomfort and finding shirts that fit is a major pain in the neck. And, I am only a double D.) But you really have to consider it. It's a major sugery that leaves scars and requires a good amount of recovery time and you may not be able to breastfeed afterwards.

That could be true about your nipples but i had a small chest and mine did the same thing. I think it really depends on what you want to do if you think you need a reduction do it. I was a really small A size and i went to a large B cup i love mine. So it depends on your health and what you feel is right. Good Luck.

My sister and I both had large breasts. She had the surgery to have them reduced. She went through so much pain. It took her a long time to heal but now she's sooooo happy. I worked out quite a bit and I actually lost a couple of cup sizes doing this. I was a DD and went down to a C cup. It's a very personal decision that you have to consider carefully. I suggest exercise first to see if that doesn't help.

I personally have small breasts, but my mother is very well endowed and she has really bad back pains. When you start feeling like you're carrying around tons of extra weight and not a part of your body, consider a reduction.

Breast Reduction surgery is very safe nowadays. My cousin got her breast reduction, tummy tuck and facelift surgery in India by the company called Forerunners Healthcare .The Price for the surgery she paid in India was very less. She paid 30% of the cost she was quoted in America.
Forerunners Healthcare is very famous in India. They arrange cosmetic and plastic surgery for foreigners in India. I read a lot about them in the Newspapers and about their patient stories. They arrange financing for USA, Canadian, UK and other international patients who plan to have surgery abroad for low price, as the breast reduction, tummy tuck, facelift and other cosmetic surgery is not covered by insurance. They also have photos pasted of their International patients. You can checkout their website. There are huge cost savings. As a doctor I personally believe that surgery can be easily handled in India, as the quality of healthcare available In India is simply best in the world. The surgeons are USA/UK trained and facilities are 5 star.

Hope this helps.
