Awww that's so sweet. Are you shy to tell her in person? Shy guys are so adorable.
Anyway, I'm one year ahead of you in school, so I know where you're at. There's not anything in particular you need to include. Just be honest. If you want, mention what you like about her and how long you've had these feelings. Don't worry about sounding weird or being too cheesy. You seem like a nice guy, and chances are, she'll think it's sweet that you went to the trouble to write her a note at all. Just make sure not to act awkward when you see her after giving her the not. Be prepared to talk about it and accept her feelings, whatever they may be.
Good luck!

Oh yeah, and don't listen to these people who are giving you trouble about wanting to give her a note. I understand that it's easier to get your thoughts across in writing, and it makes it easier to be more clear about what you want. If this girl is anything like me, she'll really appreciate it and think it's cute and sweet. <3

You need to clarify your question a little bit. Are you asking which of the three things listed should be written in your note to her or are you just telling us these things to help clarify what you're asking? If you are asking the latter, be a little more descriptive as to what you want....Better yet, forget what I just told you and ask her in person, that's just lame of you to ask her through a note.

hmm....well, honestly, i dont think you should write a letter to ask a girl out...you're in the 9th grade...not the 5th....if you're wanting to write her a letter, just write her a letter telling her how you feel, and tell her that you would like to take her out sometime. Then, get together with her, hang out, have fun, try to learn things about her. Then, IN PERSON, ask her out. Girls like it WAY better when a guy asks her out, it makes him seem *brave*.

asking a girl out through a note is so 7th grade, or Leave it to Beaver style, get the girl alone, and ask her... Hey would you like to go to the movies with me this Friday? or to some place.. have a plan, a specific movie, a specific place to go.. you can't use a note with a check boxes on it. come on!
