Nothing! Sorry, but it's not appropriate.
Getting the boss a birthday present makes the boss feel beholden to the underling, making the boss feel not like a boss.
It changes the boss-employee relationship and that's why it's not appropriate. It's the boss who should reward the employees and show appreciation for hard work.

A gift isn't appropriate unless you're really good friends or if it is a group gift. I recommend just a nice email or an email card. At the most, a normal card and leave it discretely on his/her desk.

If you're on a more personal and humorous level, check out the gag websites for ideas online. I sent my boss a cookie tin filled with fake dog crap one year that had some sort of chemical sprayed on it and heat sealed to lock in the odor... It smelled worse than the real deal. He laughed his *** off. Of course I baked homeade cookies too to back them up.

A card with a handwritten note and a business pen. You can find a nice one for about $20.00. Everyone likes a nice pen for work. or you could just give him $20.00 in gift Certificate's for a local chain restaurant like Bob's. (Bob Evans). Somewhere he could eat for lunch and take either a wife or client.
