I have owned Toyotas for as long as I have been driving, and I have a 1995 Toyota Tacoma 4x4 V6 5 speed that I bought new in 1995. I think the gear problem is just the Toyota in it. I usually shift 1,3,5, but in all the Toyotas that I have ever had, the 5 has left...I recall in the one Toyota I had that 4 and 5 left, and I would just 1,3,5 it and hold it in 5...Once in a while it may stay..at least until you hit a bump...Good luck...I love the Toyotas!

your synchronizers are going out it sounds like. usually when they go bad when you let off the gas to slow for a turn or to stop it will pop out of gear. if this is the case the transmission will have to come out to repair and it is fairly costly. make sure and replace all synchronizers if you decide to repair.
