Why do you want to do this? A plastic surgeon could do it but that would be expensive. There are various silicon inserts you can buy. Or, if you are a guy, you could do a mold of your gf.

and why are you making a life-like breast form? if it is to insert into a bra, you should go purchase silicone inserts. they sell them most anywhere you buy bras. i know Victoria's Secret carries them.
