There are several reasons for why doctors have bad penmanship:
1) So many notes are taken in medical school that have to be written quickly, and thereby, very sloppily. This habit continues into the doctor's practice.
2) What sometimes might look like scribble is actually abbreviations and notation that only a pharmacist can recognize. This is usually to save the doctor some time and to give shorter directions to the pharmacist.
3) Depending on a doctor's diagnosis or instructions, a doctor will write sloppily for a patient's protection oddly enough. Sometimes doctor's realize that you may not want anyone but them to know you have a certain condition, so when they write a prescription or write in your file only the doctor and possibly a pharmacist will be able to read it. Often times, only the doctor can read a prescription so the pharmacist will call the doctor for more specific directions. If the information is very confidential, the doctor may phone in his prescription and directions to the pharmacist.
4) Also related to security, doctor's often write sloppliy and use symbols so that, for example, a prescription cannot be forged. There have been countless instances where patients have tried to forge a prescription but do not know the proper symbolisms or signature of a doctor, therefore get busted when they try to do this.
5) Sadly, doctors see so many patients a day that they lose interest in trying to write neatly. So much writing is done in a day that it is tiring and often doctors give up and try to save some time and hand cramps by writing quickly. Of course, this results in sloppy handwriting.

Because you are writing all of the time. But I have had bad penmanship my whole life because my hand tries to keep up with my brain and so I just write really quickly and it's super messy.

When they're in college/pre-med, they get into the bad habit of scribbling while taking notes (they take a bunch of notes heh).
Also, as doctors, they have to sign and write down so many things a day, they eventually get to the point where they don't care or have the patience to write neatly.

they're at the top of the food chain... no one can say, "your writing is messy, you're fired" We need the doctors anyway! They do it because they can... Oh , and don't forget, they can hide spelling mistakes, no one would ever know the doctor didn't know how to spell something, then criticize him for being stupid... Doctors are really very smart, but a silly issue of spelling would give a false signal and make people snicker, and saving health and lives is more important than getting all hung about silly spelling issues.

Because they write a huge amount of information and they don't have time to write neatly. Think about it, drs have a lot of patients to see in one day and each patient requires a notation in the chart as well as any prescriptions and other orders. If they spent more time writing patients would end up waiting longer than they do already.

Because they write an awful lot of prescriptions and notes throughout the day. They often get lazy with the writing and try to put less effort into it as a hand can get quite sore after all day of writing. It is also quicker.
