A Sweet 16 charm with her birthdate on it. You could put it on a neclace or a charm braclett.

I would have to say it depends on what she likes. Are you guys friends? Do you have some memories together? If so, you could do some of those sentimental things; girls like guys that put THOUGHT into their gifts, instead of those store-bought teddy bears or whatever. Try something like ... a homemade collage or something that might remind her of an inside joke you guys might have.
If not... then perhaps a CD of her fave band? Season DVD of her fave show? Something that shows you know at least a lil' something about her.
Those are just my two cents; I don't know your exact situation, but good luck anyways! =)

Well, be different. Start off by arriving with a lovely bouquet of flowers -- she'll love it. In terms of a present, give her a combination of items. Give her something she maybe wanted (CDs, outfit, shoes, whatever). However, also combine that with a nice dinner out as well. If you can afford it add a play or show she wanted to see as well. She'll love it and love you for it.
