Actually, it probably depends on how the Summary Plan Document, or contract, is written.

Unless required by state law, coverage or exclusion of unmarried partners (hetero or same sex) is up to the decision-makers of each company. In the midwest, I kinda doubt it would be provided.

Ask your employer or look it up in your insurance booklet.

Not sure about Ohio, but in Michigan there is commonlaw marriage where you have lived with someone of the opposite sex for 7 years. In the common law marriage you are given domestic partner benefits.

Your question implies a fallacy though, gays are not allowed domestic partner benefits in Ohio. They have one of the more extreme anti-gay partnership laws out there. Similar to Michigan.

I think Ohio changed the domestic violence law so that it no longer applies to unmarried couples. but generally I don't think Ohio recognizes domestic partnerships
