It's fine. Unless, of course, the cap's been left off.

Here's how I know:

In our house, we get big bottles of juice because my brother drinks a lot of juice (he doesn't drink water except during sports- even then, it's usually Gatorade). By big bottles, I mean the kind you buy at those buy-in-bulk warehouse stores like BJ's or Sam's Club or Costco that come in two packs (unrefridgerated section). We usually get OceanSpray Cranberry (regular, crangrape or cranraspberry) and Mott's Apple Juice.

The bottles we get are big enough so that they will not fit in the fridge comfortably, especially if it's freshly stocked. Therefore, we leave them on the counter after they are opened, tightly closed of course.

So, If you bought the juice from the unrefridgerated drinks section of the store, then it's fine to leave outside the fridge for quite a while. The juice in our house only lasts for about a week to a week and a half, so I can't say if you can leave juice out for longer. However, if you noticed any black spots forming inside the bottle, it could be mold. Throw it out right away.

no its not good to drink that juice because after you open a bottle of apple juice its should be refrigerated and if someone had drunk from that the bottle its not safe because you may not know what diseases that person may contain

Nope; the minute you opened the bottle three days ago, room air got in, and all kinds of stuff like spores and bacteria got in with it. When someone drank from the bottle, they added tons more bacteria, fungi, etc. Sitting at room temperature, you've given them all a lovely environment to grow; if any of them were pathogens (stuff that will make you sick), then there's an excellent chance that the populations are ample to make you sick, even if you can't see them.
