Its a combination of words and actions.
He acts lovingly: and that is not just giving you flowers, chocolates, cuddly toys (that is cute, but not wholly indicative of love). Acting lovingly means respecting you as a person, act lovingly towards you and what you value, listening to you, giving you space and time you need to be you and have your life yet share in it, etc.
He also needs to say the 3 words. But saying them on their own without supporting action, and showing loving action without the words is simply not enough.

Your heart tells you everything you need to know. If you have to wonder about his love, well, he doesn't really love you.
