I have a '91 Lincoln Continental. Sometimes when i´m on the gas pedal, the car shudders for maybe 1-2 seconds. I don't notice it so much when i let the car "free-ride" downhill. What can this be? Also, (i do NOT know if it's related) the transmission seems to be very "indecisive" about what gear to stay in.

sounds like you need a tune up a bad plug could cause a miss under load

I think the 2 are related which gear are you in when it shudders probably in overdrive and when you accelerate it has a hard time grabbing the clutch in the trans. If it does this as soon as you take off then it might be something in the engine like fuel injectors or it needs a tuneup. It is common for the transmissions to go out on these cars so I would start with that.

The cheap and simple cure for this is to cut the tcc wire at the transmission. This will fix this problem. It is the torque convertor lock up wire, it will not affect the transmission as far as normal shifting and will take care of the shuddering. Or a total rebuild will have to be done. I have done this before.

hmmm.....Check the fuel delivery system. May be a fuel injection issue. If you stab on it, does it smooth out. Does it smooth out only at high rpm's?
