Check under "Department of Human Services"(DHS). There may also be a separate "Government" section in your phone book aside from the yellow and white pages.

Look in the Government pages for Human Services or Social Security office. Call and tell them you need to apply for assistance. They will guide you from there.

It should be under the government pages under Department of Human Services- DHS, I think that's what they're called now.
