They are actually one in the same...normally. Since the main dish of any meal, whether it be 2 courses or 20 courses, is usually the entree,one would tag that plate with the main dish label. HOWEVER, in some meals, your main dish can be your appetizer, soup, salad, intermezzo, etc. So although 99.9% of the time, your main dish is your entree, you can technically designate whatever part of the meal you wish as your main dish.(you like that dr. seuss rhyme at the end?). This is at least the american view.

The "main dish" assumes a meal comprised of courses. The main dish may also be served, in a line of courses, accompanied with side dishes which are also considered part of the main course.
An entree is formally detached from a course oriented meal and usually comes unaccompanied with side dishes.

In America they call the main course the entree. In Europe though, we know that the entree is the appetiser.
