They don't have to be alcoholic. The non-alcoholic versions are called "Virgin Piña Coladas."

Here is information from Wikipedia:
"Piña Colada is a sweet, rum-based cocktail. It contains light rum, coconut cream and pineapple juice blended with crushed ice. Two typical garnishes are a pineapple wedge and a maraschino cherry. Like most popular mixed drinks, the Piña Colada has variations that deviate somewhat from the basic recipe. Omitting the rum results in a Virgin Piña Colada. Some recipes use dark rum in addition to, or instead of, light rum. Others add dairy cream. Many bartenders use both light and dark rum, a dash of bitters and a little double cream, in addition to the garnishes to create a more complex flavor. A tall hurricane glass is used to pour a Piña Colada. A Chi Chi is identical to a Piña Colada, except that vodka is substituted for rum."

Yes usually they are, here is a recipe.

Pina Colada recipe

3 oz light rum
3 tbsp coconut milk
3 tbsp crushed pineapples

Put all ingredients into an electric blender with 2 cups of crushed ice. Blend at a high speed for a short length of time. Strain into a collins glass and serve with a straw.

Pina coladas usually have rum in them but you can obviously make it without the rum. Though then you'd only have pineapple juice with coconut syrup, really!

The mixes are not, you add your own according to taste. They do sell a Pre-Mix and they ready to go, and some are good and have a good alcohol content. Usually it will say on the bottle like 7% Alcohol by Volume.
