Yes one should excuse themselves, and no you are not suppose to reply with God Bless You, that is for a sneeze for you stop breathing for a second. Maybe you could say Thank You God for allowing one to expell all those toxic gases from their body.

The person farting should say "Excuse this blessed fart God has given me" and you are supposed to say "Yes, it is indeed a blessed fart."
