Santo = a masculine saint, without a proper name following it.
San = masculine saint, followed by a proper name, e.g. San Francisco.
Santa = a female saint, with or without a proper name following it, e.g. Santa Barbara.

No. San is used as the followng, like part of a name,
"San Pedro......"
"Saint Peter"
and santo means the noun
"El es un santo"
"He's a saint"

We say San, if the name begins with a consonant, as in "San José"
and Santo, if it begins with a vowel, as in "Santo António"
