If you are 'extremely embarrassed' then you might ask why, but if you are just nervous about asking some one out it is just life. Confidence is the key. Problem is that those with too much confidence dont take dating very seriously (some dont).
I would say that some things that help me is to think about what you are going to say. But right before you ask her out do some things that are normal for you. That way you are not thinking about it right before you ask, that is a problem as over thinking an issue (especially this one) can do more harm then good. Take a deep breath right before you ask (sounds silly but it will calm you). Then ask.
I have not done this but practicing with someone you trust I hear is helpful too. Good luck and remember everyone gets nervous about such things.

JUST DO IT!!!!!! If she says "no", still be friendly with her, but if you don't ask, you won't give her the opportunity to say YES. A guy I grew up with had a serious crush on me since junior high school. I knew it, but whenever I came around, he would become intimidated and quiet. He finally got in touch with me YEARS later and told me about the crush. If he would've just asked, I would've said yes. But it's too late now, I'm off the market.

Just imagine if they said 'no thanks' think of how you will feel after. At least you will know. Then sock em in the head.
