Work both your abs and back. For abs you can do basic crunchs for upper, lower and obliques. For the back: lay on your stomach and place hands under chin. Sucking your belly in, exhale and raise your upper body up off floor and hold for 3 counts and lower, repeat. Another one you can do: on your hands and knees, looking down keeping head inline with spine, lift one arm straight out and the opposite leg, hold for 3 counts and lower, repeat on other side and continue to alternate for 8 on each side. Any time you are sitting, hold your abs in - anywhere, in the car, at the kitchen table, everywhere! Abs in and shoulders back are the key! Your back may actually hurt a bit in the beginning, but that's because you are working muscles that aren't use to it.

yoga or pilates both really help.
but also, being conscious of your posture (like when you are sitting at your computer don't slouch) is also a great exercise. just being able to notice your posture helps you correct it. if you work on practicing good posture your body will eventually develop the muscle memory so that it becomes natural.

Strengthen your stomach muscles. Seriously, this will help you stand up straighter, thus improving posture.

Practice yoga asanas daily for one year and you are bound to get good results. It's no magic bullet, but it will work.
