These are extremly heavy patients on Medi-Cal and they only get one cleaning a year.

Are you looking for justification to quit or what? You have asked several versions of the same basic question when you already know the right answer. Just get out of there while you still have some integrity and pride left intact.

I dont really understand your question?? I my practice I see anywhere from 8-10 patients a day all that need scaling.. however the scaling required for a particular patient is on an individual basis. Some one that has that much build up, I would scale sextants not quadrants and scale to completion. Gross debridement is not helping your patient at all. In fact your are causing more damage by doing this, then picking a smaller area and completely cleaning it. I have spent 1/2hour-45 mins on sextant 5 before....maybe more time spent with OHI???

4 a day is my personal max, at between 1 and 2 hours for 2 quads. What about a gross debridement with ultrasonics for the whole mouth, takes 45 minutes, the heck with SRP on patients like those.

The office I work for will usually only schedule 2 per session, but I'll do 3 if time permits. They are also really good about giving me enough time, about 1 1/2 hrs. I too like the ultrasonic. I'll blast the tough stuff off with that first then go back and use hand instruments.

Whoo! normaly to 3-4 a day...or your hands/wrists will pay the price later in life!!! and depending on the instumentation about 45min..can you use an ultrasonic?
