Try these study tips:
Put on some non-distracting music. Such as classical(It's been proven to help!)
Chew sugarless gum while studying.
Eat a lot of protein the night of the studying and the morning of the test. Also, have complex carbs(such as bread) the morning of the test!
While studying, have a tablespoon of peanut butter or so.
After about a 1/2 hour of studying, take a break for 10-15 minutes. Quiz yourself over what you'd just studied, and then move on.
Or have someone else quiz you.
Encourage yourself by saying positive things to yourself...such as "I can do this." "I know this." "I'm proud of myself for studying this hard."
And try not to be anxious while taking the test. If you don't know an answer, move on..and then come back to it later.
REMEMBER: CHEW SUGARLESS GUM DURING THE ACTUAL EXAM! It lets the mind think more freely, and helps retain memory!

Lots of people have given you good study tips. If those tips don't help you might want to check with a counselor and doctor for a learning disorder.
My brother always had trouble in school. No matter what he did it just wasn't enough and he finally gave up. After a stint in the Army he gave college a try. He was still having problems. He saw counselor at the school. It was discoverd that he had dyslexia. Which basically effects how your brain recieves and relays information. For example, your brain knows the answer but it can't get that information to the hand to write. There's a disconnect. My brother who was once a solid D student before the diagnosis became a student of honor and a doctor after diagnosis and treatment.
So try the study tips. If that doesn't work don't give up. See a counselor or a doctor they may be able to help.

Preparing for exam is not only cramming of the answers or solving the sums but it needs mental preparation also.
You should know that how to concentrate while reading and writing. How to keep control on anxiety? Most the time it has been found out that the anxiety level rises so high that even answer is known, you are not able to present it nicely.
If you can devote few minutes for breathing exercises before reading and writing you will able to perform many times better.

The problem with you ist seems is lack of confidence. You have not mentioned you are in which class. School or college. Anyway, what you need is not just memorising but clear understanding. What you hear you may forget. What you see you recognise. What you practice you never forget. So write down your questions and answers periodically. Have mock tests at home. See how you score and where you commit mistakes, what are your weak areas. Try to concentrate those areas. If you are committing silly spelling mistakes, or due to oversight or overconfidence doing maths problems wrong, even though you know, you can correct these areas. Or if you forget what you ahve read, then you need to change your method of preparation. For subjcets which need memorising, do it at early morning. Don't just go at one go. Study in phases and try to recollect and revise what you have read. All the same, have confidence, you can do it. Esay to advise, but difficult to follow. I know, but success comes only thorugh hard work. There is no easy way out. Best of Luck.

I know exactly what you mean.. I was in the same boat when at school and university.. when not in an exam, I was tops but when it came to taking an exam, I could not think.. I could not put thoughts together. I wish I could give good advice...however, the best thing is to study before but not too late.. have a good sleep... have a good breakfast and you should be ready. When I was in your shoes, I had to work 25 hours a day to get money to go to school and I did not have the luxury of relaxing, etc

Do you cram before the exam. Studies have shown that cramming is not effective. Don't study when you are tired, hungry or otherwise distracted. Some people have different learning methods. Some learn best from lectures, some from taking notes and reading them over a few times. If you are in high school, talk to your counselor about your study methods, if in college, talk to a psychology professor and they can help determine what method would help you study best.

i know exactly how u feel.i hav to write my boards at the end of this year.
the best way to tackle this prolem might be to start studyin frm before n dnt mug nething.understand a topic n then u will remember it.while doin it visualize it.it will help u more.

I think your problem is that you keep reading the same thing over and over, dont do that, just read the 1s you cant get into your mind and also getting up at like 6 and reading in fresh air helps a lot.

how do you decide that you were fully prepared? why do you not score well? did you not find the time to be enough or you knew the solution but still did some mistakes or what? you have to analyse this also.
