If she is not in love with anybody now then you just go simply around her and say sorry for your breakup and then try to request and propose her to get in relationshipe with you again. I think this will work.
But if she is in love with another guy this idea will not work at all.

That depends on why you broke up. Did she leave you or did you leave her. If you left her you would have to justify why you did that. If she left you you'd have to understand why and perhaps you can ask what you need to do differently if you were to get back together.
