By doing right always... some people will be happy and gratified by your being right.. i.e. your parents, your teachers, your peers and it may astonish a number of others who did not have confidence in your knowledge.. thus when you were right, they could not believe that you had the common sense/and/or the insight to know the right answer or the right way

Some people expect you to always do the right thing, there for they are gratified to see you doing so. But others will be astonished that you do the right thing always because they don't have such expectations of you

Doing the right thing is always advisable, but I believe Twain's wit is pointing out that some cynics will be astonished, and the optimists will be gratified.
Much like the old saying, An optimist assumes that this is the best of all possible worlds, the pessimist fears that the optimist is right.

In this case, it means that if you always do the right thing, you are certain to make some sort of impression. Some will be pleased and the rest will be amazed. This is because by always doing right, you are doing the unexpected. The unexpected gets noticed.

it probably means that you should always do the right thing and it will help out some people. at the same time it will surprise everyone else, because they either don't believe it is the right thing to do, or they don't believe you would do such a thing. nevertheless you should do the right thing.
