I don't like when people use the excuse they were drunk for cheating. It's not a good one. Yes you should tell him, because he's the one who's going to get hurt when he finds out from someone other than you. Obviously you were attracted to his sister, so see what happens with that.

I think you should tell him. I have a question for you. Are you into women also or was it a one time thing? I still think you should tell him. Maybe he will get angry, for a little while, but if he truly loves you he will forgive you. But remember getting drunk is no excuse.

I agree with some of the other people about using alcohol as an excuse for what they do. You should tell him. If you don't then his sister would. It would be better if he heard it from you rather than her. If you care about him you should be honest with him.
