How did "they" trace sickness to ecoli, then to spinach for only a few hundred people?

First, the doctors did tests to find out what bacteria was in the sick people's system...they had e-Coli. You can test for this, just like you can test for strep throat..

The doctors then asked all of the sick people what they had recently eaten.

The one thing that they all had in common was spinach. Therefore, the e-Coli must have been delivered by way of the spinach.

Pretty much simple deduction.

by asking the people questions like what have they ate lately?
