Think of it this way. The head of his penis is the most sensitive part -quite possibly of his whole body- because of the amount of nerve endings, so yes, of course it will be sore.
But there is no pleasure without pain! If he wants to get it done there is no reason he shouldn't. It might increase his sexual pleasure, and yours afterall.
I don't know personally, but there's a possibility that the piercer could use a nitrogen-based anaesthetic so he doesn't feel it. Of course, no piercings usually hurt to get done, they just hurt a lot afterwards. With proper care he shouldn't have too much trouble!
What another person said... they will stick a needle down his urethra. From what I've heard, this isn't painful, just uncomfortable!

My ex-boyf had a 'Prince Albert' piercing... had very few probs... genital piercings heal quickly too.. although be careful with sex etc, until you know it's definitely, definitely healed...
Only thing is, he had to pee sitting down, cause the pee just sprayed out sideways (sorry) as a P.A goes thru the eye of the c*ck... No idea if this is a permanent thing (as I say, he's an ex).

It will take about a month to heal!
When my ex and I were together he got his done, he said it didnt hurt too bad, but healing was a bit of a problem. No sex for atleast a month for not only healing times but you dont want to cause an infection. He needs to know that they shove this thing in the hole of his penis that is about the size of a pencil. Then they pierce it. Its not a piercing for the faint of heart.

That area is a like a sponge, it fills with blood to function. If you pierce it, it may not function properly ever again. Try pressing a sharp pin into it, and you will find out that it is excruciatingly sore!

That would be his penis or his scrotum, correct? It does take some time to heal, and if not properly cared for can become an infected mess....however plenty of guys have various genital piercings and seem to like them...
Just tell him to care for it as advised by the piercer, and it should be fine...

All piercings are painful to a certain extent. More so in delicate places like that and more prone to getting an infection too.

dear friend, why in hell do you want to pierce you manhood?
it is the most painful thing to do plus it leads to sores that dont heal very easly. the other thing is when you get a erection the ring or what ever you going to put down there gets in the way and gets stuck between your flies and under wear. so please do not do it.......
