I would be weary of "unique" ideas if the bride has registered somewhere. As a wedding planner and recent bride, I had many people buy things that weren't on the registry, and while I appreciated them, I didn't need them or have any use for them. Registries are created to show what the bride and groom REALLY want and need.

However, if you still insist, or the bride doesn't have a registry, ask her mom or someone in the wedding party if they have their cake knife and server yet. You can buy those at places like Things Remembered (have it online too) and other gift specialty shops. They can personalize the server with their names and dates. They can then USE them at the wedding!!

Or, go with something like a personalized picture frame. Every bride needs lots of picture frames after the wedding. You can get it engraved with their names and the date of the wedding as well. Or, you can just make it say "The Smith Family" on it...or even,

The Smith Family

You will have to coordinate with all those that will be in attendance, but pick up some pretty recipe cards and give them to each person/family. Instruct them to write down their favorite recipe and return the card back to you prior to wedding day or at the reception. In the meantime, obtain a recipe holder (they look like binders, but much prettier) and insert the cards by section (i.e. Appetizer, Soups, Sandwiches, Main Course, Desserts). You can create your own categories, of course. The gift is you giving the entire book, but in many ways its a gift from everyone that helped make your friend's day special.

What about gift certificates for a manicure, pedicure or a massage?
She'll be so busy will those kids that she may forget to take time for herself. You can remind her. Get her something that she can indulge in. :)

A beautiful, leather-bound photo album wouldn't be bad either. Maybe with the word "Family" embossed on it.

How about something family oriented like some family pass to the zoo, museum or a gift certificate to a restaurant where they can take the kids.

Why does it always have to be unique.... not personally directed at you... but every time I read a gift thing for a wedding it's always UNIQUE UNIQUE.
How about Practical!?
I would go with the registry if she has one.

why not get her a goodie bag of "kid stuff" kinda like a survival kit for kids....disposable camera- to have to capture those moments where she was like "you should have been there", band aids, tissues, crayons, coloring books,
I know that was corny but it's kinda cute, maybe also include a book of what you learned, gather info from other moms to give her advice, especially if the kids are younger.
